The Registry is a central and confidential record of Pap tests and related follow-up test results for all Manitoba women.

The Registry will contain:

• Your name, address and date of birth
• Your Personal Health Insurance Number (PHIN) and your Manitoba Medical Number (MHSC#).
• The dates(s) of your tests(s) and all results.
• The name, address and provider number of the doctor or health provider who did your test(s).
• The name of the laboratory where each smear was read.
