Evaluation
 Baseline surveys of care providers in both primary health care and cancer care centres were completed between June 2004 and September 2004 in order to determine perceptions, attitudes, and practices of working with each other. A follow-up survey was done in June 2005, with another completed in the spring of 2007. Data from the surveys are capturing changes in perceptions, attitudes and practices. A literature review was completed to identify research tools to assist us in evaluating the patient experience as a part of the UPCON Network. Qualitative evaluation was completed in 2006 to examine the experience patients care for in the UPCON clinics as well as the experience of the Lead Clinicians and their colleagues in the project. |
