Uniting Primary Care and Oncology: The UPCON Network



The UPCON Network began as a collaborative partnership between 12 Winnipeg family practice clinics/primary health care centres and CancerCare Manitoba (CCMB) and is dedicated to promoting and supporting the shared care of the cancer patient. As of November 2011, there are 41 clinics (24 in Winnipeg and 17 in rural/northern regions).

Using a technology and education strategy, UPCON supports more than 350 primary care providers in communicating more easily about patients with cancer care specialists. UPCON is ensuring that people with cancer in our partner clinics experience better coordination of their care between their different care providers.

UPCON was officially launched on April 1, 2004, supported by a grant from the federal Primary Health Care Transition Fund, though the support of Manitoba Health. The Network has enhanced partnerships between CCMB and the Winnipeg primary health care community, with the end result of better patient care.

To that end, a proposal to evaluate and to expand beyond the initial urban boundaries into a provincial network was drafted. Support for the UPCON Network in 2006-2007 was generously provided by the CancerCare Manitoba Foundation (CCMF).

In 2007, the UPCON became a program of CCMB and currently supports two full-time and one half-time staff and a CCMF grant for educational and communication programming.
